THE CLINIC IS OPEN FOR BOTH IN PERSON AND TELEHEALTH VISITS

Dear patients,

I hope all of you have stayed healthy and well during this unprecedented health crisis. To ensure the health and safety of our community, I am offering Video Consultations as well as in-clinic visits. . If you would like to be schedule an appointment, please call me at (650) 947-0257..

If you do visit the clinic in the upcoming weeks, please review the policies and patient responsibilities below that we have put into place to ensure your safety when visiting the clinic.

CLINIC POLICIES:

Clinic visits will be limited to one-on-one sessions.
Upon entering the clinic and when leaving the clinic, everyone will wash their hands or use hand sanitizer which is provided at the front desk.
The physical therapist will wear gloves during the entire visit which will be disposed of between each patient visit.
Patient and clinician will maintain a six foot distance when possible.

PATIENT RESPONSIBILITIES:

Please notify the physical therapist if you have been in the clinic recently and have experienced symptoms or been diagnosed with Covid-19.
Reduce physical contact and stay six feet away from others while in the clinic.
Please utilize the telehealth platform for video consultations.
